My suggestion is to make your reservations as soon as you board. There probably won't be any problem in getting a reservation but it's better to be preemptive.
In regard to the cost. Your cover charge covers almost everything except liquor and sodas. There "may" be a specialty item that is an extra charge but the vast majority of items will be included.
