medical authorization form if you're leaving your kiddies home while cruising

anyone else leaving kiddies behind? here's a medical authorization form so that whoever is watching them has authorization incase of an emergency. hopefully no one will need it but just incase. and don't forget to leave their insurance cards as well

If I can add something to your post. The medical ltr must have a notary. We carry medical power of attorneys for our three grandkids. We keep them in our cars glove boxes. Ours is from our lawyers office.
