Default Guardian Consent

My husband and I are leaving our daughter in the care of my family when we cruise. Have any of your ever filled out a Guardian Consent form? If so, where did you get your form? I have found one, but would like to compare it to others. We are leaving in 24 days --wooooohooooo!!!!!-- and I am trying to get my ducks in a row.

As long as you have it in writing that between the dates of ..... and .... , so and so( name address & phone number) have our consent to seek medical attention for our son/daughter - name.
You and your spouse must sign it and have it notarized. Leave a copy of your insurance card with the family that is taking care of your children.

Having it notarized is important. Without it, the hospital may choice to not treat your child unless it is a life and death situation. My SIL was a pediatric nurse in a hopital and she said they did NOT honor those notes unless they were notarized. So if your child had an ear infection, they may not treat it.
