You don't have to have a copy of the stock certificate (Most people don't have the actual certificate). What you need is a copy of your brokerage firm statement showing that you own Carnival stock or a copy of your voter form. If you book through a local TA just give them the copy. If you booked direct, fax the copy to Carnival.
I think you get $100.00 per cabin OBC.

It is true that you will get on board credit for being a stock holder with Carnival (100 shares minimum). The amount depends on the length of the cruise, so far what I've seen is $100 max.
The cruise lines under the Carnival corporate umbrella also can receive on board credit, Princess, HAL etc.
There is no form or information on the Carnival site, will need to call the respective line and find out what is needed to mail in for the credit.
