Default Bringing sons teen friend, what documents will we need?

We plan on taking our sons friend along (age 15), and I already know he has a passport - I found in posts here somewhere a document that I worked from basically stating "we are taking him via plane..to travel on Liberty...dates etc. and plan on having this document notarized by both parents (they are dicorced.)
2.) Authoirzation to travel notarized
anything else??

I think you already have everything you need, but here's a copy of the Consent for a Minor child to travel (which you probably have):

to any necessary routine or emergency medical treatment during the aforementioned trip.

The only other thing I can think of is a credit card or cash for your son's friend. Otherwise, I think you're covered! Have a great cruise, and what a lucky friend!

Fern, thanks for the form- the one I located resembles it, but I like this one better. I knew there was something I overlooked -by April I may have remebered --medical permission!
