Hi Lipz,
Been to Cozumel a few times, had never taken spanish and never had a problem. Its very American there, they readily accept the American dollar as well.

You really do not need to know any spanish in Cozumel. It is nice to know a few simple phrases however. Go to cozumelinsider.com they have a "refresher" course on the site that gives you some basics. I feel it is best to at least know how to communicate things such as How Much? How long? for taking a cab.

No need to speak Spanish in Cozumel! You'll be just fine only speaking English!

ALSO ... The water (most important ICE) is purified in Cozumel restaurants so this is not a problem either. Eat and Drink whatever you wish in Cozumel without a worry!

Have Fun! WSEA
