We have booked our first cruise and we are so excited. We leave out of the Galveston port on the Carnival Conquest. We will have port stops in Montego Bay, Grand Cayman and Cozumel. I would really appreciate any and all advise for us first-timers as to what to take and what excursions you really enjoyed.
My own personal recommendations for your ports of call
Cozumel.. rent a vehicle, drive around the island (it's small and tough to get lost) and stop at one of the many beaches along the way.
Grand Cayman... for your first visit you have to experience Sting Ray City
Montego Bay... its a bit harder to reach than from Ocho Rios but climbing Dunn's River Falls is one of the "musts".
There is also river rafting from there, along with the canopy tour, both at the same place. Its a good drive from the pier, but there is absolutly nothing to do by the pier, so your best bet is to book an excursion from Montego Bay.

Montego Bay - grab a cab at the pier and head over to Dr's Cove Beach, short cab ride, really pretty beach, and cheap too, Marguertiaville is basically next door, make round-trip taxi arrangements. Dunns River Falls is great fun but its a good hour ride each way- book a ships tour if you decide to do this.
Cozumel - if you snorkel or want to try this is the port to do that, we always book a snorkel tour out to the Palencaar Reef. Another great day activity is to grab a cab and head over to Channunkaab National Park- lots to do-beach, aquarium, ruin replica, dolphin swims.
Grand Caymen - a trip out to Sting Ray City is a must do for first-timers, i highly recommend booking with Captain Marvin, very reputable
