Next to the bottom of your picture it will say "Passport Agency" and below it will state the city your passport was issued in. Mine says Chicago and hubby is Canadian, so his says Hull, Ontario. Hope that helps.
Not any more! We just got our new passports in June of this year, and there is no spot that shows where ours, at least, were issued. Our originals (now expired) say "Chicago", but our new ones don't even have a spot that says "Passport Agency". And I LOOKED! <G>
So, to answer the OP question, when the Carnival funpass asked for a city, I simply put in "United States"....and it seemed to be acceptable.
BTW, don't forget to go to page 7 of your passport and put your personal information there - in pencil!
I didn't know to fill in page 7 so will do that too. Will try to just list united states and see what happens. Did you get one of the new ones that are bar coded? We got ours is August and got the older style.
We got ours in June, and since we live in Missouri, they would have come out of the (probably now-defunct) New Orleans office. They are the "old style" - no bar coding or microchipping on these babies. But they're still good until 2015 - LOTS of opportunity for stamps from various countries!
I can answer that one ... I applied for my passport on 07.26 was told I would receive it about 09.04 ... then Katrina hit ... was told to wait a couple of weeks then call NPIC (National Passport Information Center) I had to go and reapply for my passport, only had to pay for new pics and to have the Clerk of Courts send it my US post overnight ... it was address "Attention Katrina" ... this was 09.13 ... I am still waiting to get it ... If I do not get it in the mail tomorrow (Monday) I will give them a call ...
