If the insurance is purchased within the 7 - 21 day period you receive two enhancements (varies by company and plan) to the policy.
1. protection against the financial default of a tour operator, airline or cruise line. (depends on company)
2. the pre-existing medical condition exclusion is usually waived (again, depends on company and plan).
Most companies count the day you make the first trip payment as day one (not the day the check is cashed).

DO NOT GET CARNIVAL INSURANCE. If there is a problem, you will need the insurance company to go to bat for you. Since that would most likely be Carnival, you have lost your batter, so you would go battie.
Well crud, we bought the insurance through Carnival. One of the hazards of being a newbie I guess. We booked through a local TA and she told us we had to decide right then if we wanted the insurance or not. Naturally I wanted the insurance, but felt pressured to go on and get it. I think next time I will book through Carnival or some other online TA and check with insuremycruise.com. live and learn.
Check with a TA. I know the insurance we sell at my company you can purchase up 2 days prior to sailing.. However it won't cover any pre existing conditions. (pre existing - Any change in medical in the last four months.)
