Default Notorized Letters

I know that I have talked with fellow cruisers regarding traveling with children who are not your own. In four weeks we leave on the Imagination. We are taking our son and his 15 year old friend. I told his dad that I would come up with the letter and then he could have it notorized. My son's friends mom died a couple of years ago and she handled everything back then.

Does anyone have an example copy that I could go off of? By the way, he does not have a passport for this trip. We do though.

I know that I need to hit the highlights:

Permission to take him out of the country and dates.
His full name and father's full name.
Permission to get medical attention.
They don't have insurance so I can't get medical insurance card.
Name of ship and dates.
Where we are going ?
Do we need to explain in the letter that the mother is no longer with us?

THanks so much for any help that we could get. I'm getting down to the wire on this and I want to go ahead and get all the paperwork done.

We took our daughters friend a couple years ago. Our letter was similar to the one in the previous reply. Just some advice, make sure you check with the parents that the child doesn't have any food allergies. Our daughters friend has a food allergy to shellfish and I didn't think about that. Her mother told me a couple days before we left.
Luckily, Carnival was great in helping us with this. We went to the pursers desk and they let her plan all her dining room meals for the week. They already knew what to bring her when we got to the dining room.
Hats off the Carnival for taking care of their passengers.
