I'd love to help 6 nice people from the Netherlands. I want to make sure I understand your question though. Are you asking why they don't offer a transfer from Ft. Lauderdale to Miami, because they offer transfer service from Ft. Lauderdale to Port Everglades?
If that's your question, it is probably because Ft. Lauderdale's airport is only about 5 minutes from Pt. Everglades. Miami is quite a bit further away.
If it were me I would probably rent a van in Fort Lauderdale and drive it to Miami, since there are free shuttles at Port Everglades that will take you to the rental car agencies, I think Dollar and Thrifty come to mind. You could also rent a GPS if you are uncertain of where to go. You can then drop the van off at the Miami airport and go catch your flight. I have gotten good rates from carrentals.com
The only reason I would hesitate to recommend doing a rental car is based on your comfort level driving in a foriegn country. Last month we drove from Belfast to Dublin Ireland and it was a lot of fun, but if we did not have the GPS, we'd probably still be on one of the many roundabouts like the Griswold family on family vacation.

It will be open when you first get to your room. If you have problems, just ask your room steward. You will likely use a credit card (not your room key) or another card with a magnetic key to open it.
It is important that you do not use your key card for the safe. It will actually invalidate the key card and you will find yourself in a long line at the Pursers desk.

Carnival does offer a nice tour of Miami. I took it in May 2007. We toured the Art Deco section of the area where a number of films were made.
In the 2008 Carnival shore excursions guide they do list ....."EVERGLADES TOUR: Ideal for guest with flights after 3pm from Miami International Airport"
