If it's your first visit to any of the destination cities, I highly recommend the shore excursions that visit the major highlights of the destination city. Also, in Barcelona, the shore excursion that combines a city drive with a tour of Montserrat is excellent.
In Naples, the shore excursion that goes to Capri for the whole day is excellent. In the town of Anacapri, be sure to watch the demonstration of the gorgeous wood inlay game table at the department store (which is directly across the town square from the main road).
If you have been to a destination city that's some distance from the respective port of call before, the "... on Your Own" excursions are an excellent option. These excursions, which provide only bus transportation between the pier and the downtown of the destination city, are the most seamless way to get to the city if you want to shop, take in the scene at a local cafe, or explore on your own.
Finally, I should mention that many of the shore excursions in the Mediterranean region visit churches and other houses of worship that have very strict dress codes because those houses of worship also house very significant works of art. Basically, you'll need to wear clothing that completely covers your shoulders, your knees, and everything in between. The churches also require that ladies cover their heads with a hat, a scarf, a kerchief, or something of the sort.
