Spa Services- is everything added cost?

I've only ever set foot in a spa twice for massage or hair styling services.
Are there any services that are included in your fare?
Are there indoor pools that anyone can use, or whirlpools/hot-tubs without added cost? (in the spa, not the on-deck whirl-pools)

Gym areas and some pools are free. The S class ships have a great indoor pool area by the spa with a free Aqua Cafe with healthy food. Most of the services in the spa however have a fee service.
