You have the choice of wearing underpants, but most people do shed all their clothes, but they are draped in sheets and are shielded when changing positions (ie: going from lying on stomach to lying on back or vice versa.)

They have always told me to undress to the level I am comfortable with. Some people leave their underwear on and some don't. You don't want to leave on too much because it will get pretty slimy from the massage oil.
The massage I had on Carnival was wonderful. They gave me paper underwear to put on under my robe. Then it's up on the table, take off the robe behind the sheet, and then the sheet is put on your body. Simple as that.
