For fun family activities ...
SanJuan........ just walk around Old San Juan and check out El Morro fort. Another option if u have time is a trip to the rain forrest.
St. Thomas....we once did a sail & snorkel to Buck Island with another family.. 9 of us on " our own" sailboat. Had a captain to sail her, and a first hand, who also acted as an underwater guide for snorkeling.
another recommendation would be going to over to St. John/ Trunk Bay for a nice beach day.

Another fun option in San Juan is to take a horse and carriage ride through old San Juan. One time when we called in San Juan we didn't disembark until early evening so we opted for a fun, relaxing horse ride. My daughter (six years old then) thought it was pretty exciting and I enjoyed seeing the imposing buildings all lit up by night. We walked past the casino (near the cruise ship) pier to the tourism information building which is on a small plaza, where we hooked up with the carriage rides.
If you are there by day, you may want to opt for a shore excursion to the rain forest which is good, clean family fun.
Enjoy!

In Old San Juan, there is also the free Trolley that you can get on and off at many historic sites. Take it to the top of the hill to El Morro, then the walk down will be a lot easier on everyone or just hop back on and get off where it is interesting to you. In St. Thomas, take a taxi to Maegen's Bay for some great beach time in the morning, then return to the ship in the afternoon for lunch. You could also take the Paradise Tram up the mountain right across from the Havensight port. For shopping, either plan on downtown Charlotte Amalie or right at Havensight Mall.
Bon Voyage
