Depends on the cruiseline when it comes to fridges. Princess always has a fridge. Don't know about the needles issue. I'm sure each cruiseline will do their best to try to accommodate, if you inform them well ahead.
There are always options for dieters and again notify the cruiseline ahead for any special dietary needs. I'd talk to the Maitre D upon embarkation as well.

I need to keep my insulin cold. Depends...Most do. You could bring a small cooler (six pack size) and have the cabin steward keep ice in it for you.

Yes there are "Spa" marked deserts. You can check the daily menu outside your dining room early in the day to see if they will have what you want.
