Sanibel Island is absolutely awesome; it is an area that is less touristy and the beaches are more natural etc. it is quite a drive - probably about 2 1/2 hours to get out on the island from Miami to Sanibel and now that the new bridge is done it goes a little quicker. you will not find typical chain hotels on Sanibel but there are very nice smaller, locally run operations; I have not stayed on the island but work out there every summer and it is beautiful.
to further answer your question: Sanibel beaches; = quiet, relaxing and natural settings; Miami Beaches: = a show, loud, fun and off the wall.
the cost of staying on Miami Beach vs Sanibel Island (Captiva Island) are pretty much the same; neither one is cheap.
