I have recently bought tickets for my wife and sister and I to travel the Med. Sea on the Brilliance of the Sea. The booking gave us a small option for 3 people rooms, however, I notice that most pictures only have two twin beds with a couch. The last cruise we went on, the couch was not a comfty couch. Does anyone know exactly how they sleep 3 in these rooms?
Usually the couch will fold out into a bed. Depending on the room, they also have beds in the ceiling or in the wall that fold out. Either way, your cabin steward will take care of unfolding the bed from the wall or couch and making it up every evening, and putting it away in the morning.

Not all rooms are set up for more than 2 people. Some cabins are designate for only 2 and won't have the fold out beds in them.

if you tell them when you are booking, they can accommodate you for 3 people **depending on availability**
i sailed on Brilliance in june '08 and it was no problem with 3 of us in the room. we had plenty of space. we also took an over the door shoe hanger to put our toiletries in so we didn't run out of counter space~worked great
