I wrote up a letter similar to that, listed our names, address, SSNs, trip itinerary, contact info, etc. The child's mother is signing it and having it notarized. We carry my stepson on our health...

Same last name - he's hubby's son. I wrote up a page long thing with all our names, birthdates, SSNs, the itinerary, confirmation number, stateroom number, ship name, dates, the whole nine yards!
