We were there last year and decided against a tour. Found out that there was a straw market just down the road from where the ship docked. Easy walk, even for me. And, since this is a fairly new port to the cruise ships, the market was interesting and fun. Some true local arts and crafts.
I haven't been there in 20 years, but I would check out the other 2 beaches on Cayo Levantado - it can be a bit of an adventure to explore the island. I don't recall there being much else to do there but times may well have changed.

There are many awesome things to do in Samana but you must have a plan before you get here. Try the Zip Line, El Limon Waterfalls, ATV Quad tours, see the Whales ( Jan-March), Los Haitises (like mentioned above) or some of our other awesome secluded beaches.
