I would say it HAD to have been a major navigational error. Although charts can be outdated and sandbars can shift - a long gaping hole is not caused by a sandbar.
That ship hit something permanent and sharp - some kind of rocky shoal.
Of course I live in Phoenix so I get the news late, I just woke up.
