I recently changed my passport to my married name. I know that I went through the proper procedure (sent it to Charleston, SC), when I received it back the only way to see my changed name is by flipping to the last page of my passport where it states, effective 3/1/04 the name on this passport has been changed to "X".
Has anyone had a problem using a name-changed passport? I don't know why this concerns me, but it just seems that the name on the id page of my passport should be changed to my new name.
We recently experienced the same thing, and were also surprised that they only put an endorsement in the back of my wife's passport. We are sailng May 22, so we'll see what happens. Hopefully this is not uncommon and won't raise any eyebrows.
No worries, it doesn't cause any trouble at all. I had my name changed in the same way, and also my passport is a duplicate for a stolen one, so lots of page flipping has to be done. I've been using this new one for nearly 5 years, not a single problem.
I sent an e-mail to the State Department asking them if the back page that indicated my new name would be sufficient for my passport renewal or did I need to include my marriage certificate again. The State Dept. relied to my e-mail saying that I did not have to incude my marriage certificate again. I sent a copy of my e-mail and there reply with my old passport and the renewal form and will carry a copy with the new passport as well. It might help if someone questions your passport.
Hey i've got a question regarding passports--When you fill out the information on the application--Do you use your married name as last name?? or your maiden?? I've been married 19 years and always go by married name, however my B/C has maiden name on it??
